Patient Educator (Healthcare)
Work in close cooperation with manager, key staff and community partners to assess patient education needs and provide optimal delivery of patient education programs and services within clinic and community.
Work with the QIP Department to support goals and promotional campaigns in an effort to improve QIP initiatives.
Develops, presents, and evaluates workshops and trainings as needed.
Develops and distributes health education and promotional materials in various forms and methods, including but not limited to newsletters, display boards, social media, visual aids, video, photos, and posters, as needed. Note: There will be a special focus on health literacy. Materials review and editing will be a big part of this work.
Document patient education services as needed and appropriate in EPM/EHR.
Develops needs assessments, goals, and objectives and uses evidence-based practices, as well as the theories and models of the health education and promotion field.
Mentor lay leaders, volunteers and student workers who assist periodically with department activities under the guidance of the manager.
Work outreach events and health fairs as needed, which may include fundraising.
Performs various office/administrative duties as necessary, including preparing comprehensive reports and correspondence, compiling and analyzing statistical data for reports, attending and conducting meetings, procuring supplies, maintaining files, etc.
Attend professional development trainings, meetings, and conferences to enhance job knowledge and skills.
Encourage and work within a team, as well as have the ability to be self-directed.
Inform department manager of current trends, problems and needs to facilitate planning, development and policy-making.
Attend meetings as assigned.
Follow compliance with all relevant laws and regulations regarding confidentiality and HIPAA standards and regulations.
Adhere to legal and regulatory standards.
Act upon ethical and professional standards of the clinic and the health education and promotion field.
Duties may cross over at times to employee wellness.
Other projects as assigned.
KNOWLEDGE, SKILLS, AND ABILITIES
The Patient Educator must have the ability to work with a diverse population in a kind, caring and compassionate manner and communicate effectively with patients from a variety of backgrounds.
Excellent communication, organizational skills are required.
Skill in program planning, organization, implementation, evaluation, problem-solving and administering health education programs.
Ability to multi-task and strategize priorities, as well as willingness to adapt to change in a positive manner.
Able to skillfully lead, empower, and build on patients' strengths in an effort to navigate patient self-management and healthy behavior changes, as well as demonstrate a commitment to respectfully serve the underserved.
Knowledge of electronic medical record systems.
Computer literacy and demonstrate proficiency with word processing and spreadsheets in a Windows based environment.
Knowledge of the principles and practices of health education and its core competencies, including but not limited to a general concept of public health and preventive medicine.
Knowledge of educational techniques, with strong verbal and written communication skills.
Knowledge of community resources.
Knowledge of grant administration and reporting.
Knowledge of healthcare regulations and organization policies and procedures.
Knowledge of data collection practices and procedures.
Ability to make accurate arithmetic, financial and statistical computations.
Ability to read and interpret moderately complex materials pertaining to the responsibilities of the job, assemble and analyze information and keep clear and concise documentation.
Ability to maintain significant flexibility in daily operations and decision making, react calmly and professionally in an emergency, emotional and/or stressful situations, and practice the upmost in safety at all times.
Ability to establish and maintain effective working relationships with those contacted in the course of work.
EDUCATION & EXPERIENCE
Bachelor's Degree in health and/or education related field and minimum of 2 years working in the field.
CHES or MCHES certification desired.
A combination of education, training and experience which would provide the required knowledge, skills and abilities may be considered.
Requires a valid California driver's license.
