The primary purpose of the MDS Coordinator is to ensure the facility meets regulatory standards for resident assessments, focusing on the completion of MDS assessments and the development of resident care plans. This role involves overseeing the entire MDS assessment process, ensuring that all assessments, including PPS Medicare, quarterly, annual, and significant change assessments, are completed accurately and on time. The MDS Coordinator also ensures interdisciplinary collaboration to ensure a comprehensive and accurate assessment of each resident's health status and level of care.

Key Responsibilities:

• Oversee the facility's weekly PDPM meetings for skilled patients and track the effective utilization of PDPM tools such as Physician Diagnosis Verification, Admission/Discharge CSR(GG), and PDPM Weekly Tracking Tool.
• Monitor and analyze facility's performance on Quality Measures (QM) and Quality Assurance and Performance Improvement (QASP) on a monthly and quarterly basis, offering clinical recommendations for optimal outcomes.
• Provide PDPM and MDS training to new MDS Coordinators and ensure proper education on guidelines and protocols.
• Assess and determine the health status and level of care for all new admissions.
• Ensure the accurate and timely completion of all MDS Assessments, including PPS Medicare, quarterly, annual, and significant change assessments.
• Coordinate the interdisciplinary team's participation in completing MDS assessments for new admissions within regulatory timeframes.
• Maintain an accurate schedule for MDS assessments, ensuring proper reference dates are established throughout the resident's stay.
• Manage the data entry and electronic submission of MDS assessments, making necessary corrections and maintaining proper records.
• Schedule and conduct resident care conferences in compliance with state and federal regulations, ensuring completion of all MDS reviews prior to the conference.
• Assist in formulating and revising resident care plans, ensuring that identified problems are prioritized and appropriate interventions and goals are established.

This position requires strong attention to detail, an understanding of regulatory guidelines, and the ability to collaborate effectively with other healthcare professionals to ensure optimal care for residents.

Requirements:

Must possess a current, unencumbered, active license to practice as an LPN or RN in Washington State.

Current CPR certification.

Knowledge and experience with PCC preferred.

Must maintain all required continuing education/licensing.

Must remain in good standing with the State Board of Nursing at all times.
