What You'll Do:
✔️ Resident & Family Support: Provide emotional and psychosocial support to residents and families, ensuring their concerns are addressed and needs are met.
✔️ Care Planning & Advocacy: Assist in developing individualized care plans that enhance the well-being of our residents while promoting their independence and dignity.
✔️ Discharge Planning: Work closely with families, healthcare teams, and community resources to coordinate seamless transitions home or to other care settings.
✔️ Community Resource Coordination: Connect residents and families with essential services, such as financial assistance, housing, and mental health support.
✔️ Compliance & Documentation: Maintain accurate documentation and ensure all social services meet federal and state regulations.
✔️ Interdisciplinary Collaboration: Partner with nurses, therapists, and administrative staff to ensure holistic resident care.
